This page contains team-based training materials that were created by the Institute on Community Integration, University of Minnesota in collaboration with the Minnesota Department of Human Services. Each organization is asked to identify a team of people who represent the key roles and responsibilities within the services provided. The number of members of each team vary based on the size of the organization providing services. Usually two or more teams meet across five team training days. The materials below reflect the presentations, tools and activities that teams use to create an action plan for improving person-centered and positive support practices.
